Practice Pre-Visit Checklist for Heart Health (ABCS) Status

Primary care practice staff can fill out this one-page checklist before a patient visit to highlight for clinicians essential information about that patient’s ABCS status and flag upcoming care needs in the electronic health record. This tool is an example of how practices customize and operationalize evidence, part of Key Driver 1: Seek, select, and customize the best evidence for use by the practice. It also establishes a process for identifying patients affected by the evidence, a component of Key Driver 5: Engage with patients and families in evidence-based care and quality improvement.
Note: Although this is still a good example of how to operationalize evidence, the ABCS evidence included here may be outdated.
